HDR34 - Thesis Submission Declaration & Approval

This is to be completed by Research Degrees candidates once they are ready to submit the thesis.

An electronic PDF copy of the thesis is to be uploaded to this form.

Other documents to upload to this form include:

• HDR31 - Co-Authorship Approvals for HDR Thesis for Examinations form, if applicable, 1 form per publication

• Final iThenticate Similarity Report

• All relevant answers must be completed. 
  

• Progress on this form can be saved and completed at a later stage.

• When selecting 'save and continue later,' a link will be emailed to enable you to return and complete the form.

Workflows:

Workflows have been enabled with this form to facilitate authorisations and signatures as follows:

1. Candidate to complete sections 1 - 9. Then, submit the form, which will be sent to your principal supervisors at the address you provide in the form. (Please ensure you enter the correct email address here)

2. Your principal supervisor is then required to complete section 10 and submit it to the Dean of Graduate Studies for final review.

3. Dean of Graduate Studies to complete section 11 and submit to HDR Exams for processing.

This information could help the examiner:

• Appreciate the effort necessary to write the thesis 

• Understand the cause of any minor grammatical errors 

However, write the thesis as proficiently as you can and seek an editor if needed.

To earn this degree, you must participate in at least five (5) workshops, activities, and experiences that extend your capabilities and promulgate your research. 

If yes, 

• please list the workshops you attended in the space below, including the

  • workshop/training title

  • whether it was online or in person

  • and the date attended. 

• Number them 1–5, with each workshop on a separate line.

If you have not completed the minimum of five workshops, please list below

• under the heading "completed" those you have attended / completed and

• under the heading "planned"those you PLAN to attend/ complete before the end of your degree, including the dates.

For workshops and training that were not organised by the HDR team, please upload evidence of attendance for each workshop

To check for potential plagiarism, please run a similarity report in iThenticate on your final thesis.

Check each instance of similarity in the report to ensure all work is correctly attributed and edit if and as required to avoid any potential plagiarism. Then, generate a similarity report on the FINAL version of your thesis through iThenticate and upload it here.

Note: all domestic HDR students and any International HDR students that were in receipt of a Research Training Program (RTP) Stipend or RTP offset must include an acknowledgement of the support and the RTP DOI. Once you submit the form, an email containing the workflow link will be sent directly to your Principal Supervisor at the email address you provided above.

(Note: Please ensure that you click 'submit' for it to be sent to the Principal Supervisor)

Your Principal Supervisor will then proceed to submit the form to HDR Exams.

Please indicate which of the following statements are correct.

If the principal supervisor does not believe the thesis is ready to be submitted, but the candidate wants to submit the thesis, the Research Committee will determine whether the submission should proceed.

Please copy your Faculty/ Menzies delegates email from the list below and add it to the Faculty/School Delegate email box. 

Once you click 'submit' the form will be sent to the relevant HDR Delegate to request endorsement.
