HDR54 - Request to reinstate stipend payments

For Research Degree Candidates

Please complete this form if your stipend has been suspended and you would like payments of your stipend to recommence.

Please ensure the form is submitted at least two weeks before the recommencement of stipend payments after discussion with your supervisor.

If you would like to update your banking details please complete HDR Bank Account details form

• All relevant answers must be completed. 
  

• Progress on this form can be saved and completed at a later stage.

• When selecting 'save and continue later', a link will be emailed to you, that enables you to return to complete the form.

Workflows:

Workflows have been enabled with this form to facilitate authorisations and signatures as follows:

1. Candidate to complete section 1. Then submit the form, the form will be sent to your principal supervisors with address you provide in the form. (Please ensure you enter the correct email address here)

2. Your principal supervisor is then required to complete section 2 and submit to Research Degrees
   

3. Research Office to process request and advise Candidate by email.
