HDR11 - Change of Supervisory Panel

For Research Degree Candidates

This form is used to change the individuals on your supervisory panel and change Faculty’s.

GUIDE TO COMPLETING THIS FORM

  • All relevant answers must be completed. Depending on your response to a given answer, additional information may be required. You will be unable to continue until the required answers have been completed.

  • Progress on this form can be saved and completed at a later stage.

  • When selecting 'save and continue later', a link will be emailed to you, that enables you to return to complete the form.

Workflows:

Workflowshave been enabled with this form to facilitate authorisations and signatures as follows:

  1. Candidate completes Section 1 - submits to Principal Supervisor

  2. Principal Supervisor completes Section 2 - submits to Faculty / School administration office

  3. Faculty / School administration office completes section 3 – submits to Faculty / School Delegate

  4. Faculty / School Delegate completes section 4 – submits to Research Degrees Office

  5. Research Degrees Office completes section 5 – submits to Dean of Graduate Studies

  6. Dean of Graduate Studies completes section 6 – submits to Research Degrees Office

  7. Research Office to process request and advise Candidate by email.

A copy of the form will be sent to you once you submit the form.

Section 1a: Details of the Candidate

**If you are changing the principal superivsor on your panel - please enter your new principal's name and email here.**

Section 1b: Supervisors

Please specify 1 principal supervisor and 1 or more associate supervisors. These individuals must be regsitered supervisors who are paid by CDU or Menzies - or who dedicate substantial time to CDU or Menzies.

Total contribution to supervison panel must equal 100%. (Note: Advisors do not contribute to the supervision percentage)

 NOTE:

  1. Only registered CDU/Menzies supervisors are eligible to supervise HDR students at the Principal Supervisor or Associate Supervisor level. 

  2. If you are unsure whether a proposed supervisor is registered, please check with the Faculty Administrator before submitting the form.

  3. External supervisors must hold both: 

       a. an adjunct or honorary appointment with CDU, and 

       b. current supervisor registration with CDU.

If an external supervisor is registered but does not hold an adjunct/honorary appointment, or holds an adjunct/honorary appointment but is not a registered supervisor, they will be added to the panel as an Advisor.

Section 1c: Other members of the supervisory panel

Specify end users or industry partners, academic advisors from other universities or organisations.

Advisors must comply with CDU policies and procedures, including the Staff Code of Conduct, the Responsible Conduct of Research policy and procedure. 

PLEASE note: Australia currently has sanctions frameworks and export control measures on research collaborations with certain countries, including Iran, Russia and the Democratic People's Republic of Korea. Legislation and Sanctions Frameworks | Australian Government Department of Foreign Affairs and Trade

Advisors from sanctioned countries/ employed by organisations in those countries are prohibited except with written permission from the DVCRCC. 

There is also a consolidated list  " which includes all individuals, entities and vessels subject to Australian sanctions, including targeted financial sanctions, travel bans, arms embargos and maritime sanctions"   Consolidated List | Australian Government Department of Foreign Affairs and Trade

As such, advisors on the consolidated list or from entities on the list are prohibited. 

Section 1d: Changes to your panel reasons

1. Any changes to supervisors (addition, removal, or replacement of supervisors); 2. Any changes to the supervision percentage allocation for each supervisor; and 3. The inclusion of any advisor(s) on the supervisory panel. This information is required to support the review and approval of the proposed changes

Section 1e: Supervisory Charter and Signature

A new requirement is now in place. Please carefully read through The Charles Darwin University Supervisory Charter.

This Supervisory Charter outlines the expectations, responsibilities, and commitments of both parties, creating a solid foundation for a successful research journey. The Supervisory Charter supersedes the HDR14 - Supervision Agreement.

Any time you change your supervisory panel, you will need to confirm your agreement with the terms.

Please take the time to thoroughly review and discuss it with your supervisor(s) before signing.

Furthermore, your principal supervisor must also sign, indicating their agreement with the conditions specified in this document.

Once your supervisor has endorsed, you will receive a copy of the form.

This form then requires endorsement from the Faculty Delegate and Dean of Graduate Studies.

Once all endorsements have been received, you will receive notification from the research office.

Section 2: Endorsement from Principal Supervisor

This Supervisory Charter outlines the expectations, responsibilities, and commitments of both parties, creating a solid foundation for a successful research journey. The Supervisory Charter supersedes the HDR14 - Supervision Agreement.

Any time your candidate's supervisory panel changes, both candidate and principal supervisor will need to confirm agreement with the terms.

** Please note, this student has requested to change Faculty's - if you approve this change request - please endorse the form and the Candidate Progression Coordination will request the 2nd Faculty's endorsement.
